Who we are

Tetra Insurance Agency LLC is an independent insurance agency in Grosse Pointe Shores, Michigan. We help people shop, apply for, and service insurance coverage through the carriers and wholesale markets we have access to. We do not underwrite insurance. Carriers make underwriting and claims decisions.

This policy explains what personal information we collect, why we collect it, who we give it to, and what control you have over it. It applies to our website at thetetraplan.com, to our text messaging program, and to information you give us by phone, email, or on an insurance application.

Your privacy rights under Michigan law

As a licensed insurance producer we are subject to Chapter 5 of the Michigan Insurance Code, MCL 500.501 through 500.547, which governs the privacy of consumer financial and health information. Chapter 5A of the code governs our data security obligations. The federal Gramm-Leach-Bliley Act and the FTC Safeguards Rule at 16 CFR Part 314 also apply to us.

Information we collect

Information you give us. Name, address, phone numbers, email address, date of birth, Social Security number, marital status, household members, employment and income, the property or vehicles you want covered, existing coverage and carriers, beneficiaries, and anything else on an application.

Information from third parties. Motor vehicle records, loss history reports such as CLUE, insurance scores and credit-based information where permitted, prior carrier records, property inspection reports, and quotes and underwriting responses returned by carriers.

Health information. Life, disability, and health insurance applications require medical questions, prescription history, and sometimes medical exam results and records from your providers. This information is collected only with your signed authorization and is handled under the protections described below.

Information collected automatically on our website. IP address, browser and device type, pages viewed, and referring site. Our site uses Google Analytics and a Meta advertising pixel. These tools set cookies and may allow Google and Meta to recognize your device across sites. You can turn off cookies in your browser and you can opt out of Google Analytics using Google's browser add-on.

Information from web forms and quote tools. If you request a quote or fill out a contact form, we collect the name, email address, phone number, and coverage details you enter.

Mobile and text messaging information. Your mobile phone number and your consent to receive text messages from us, along with the content and timestamps of messages between you and us.

How we use your information

  • To shop your coverage with carriers and wholesale markets and to obtain quotes
  • To submit applications and follow them through underwriting
  • To service your policies: endorsements, renewals, billing questions, and certificates
  • To assist with claims and to communicate with carrier claims departments
  • To communicate with you, including by text message
  • To meet legal and regulatory obligations, including Michigan producer licensing rules and record retention requirements
  • To improve our website and measure the performance of our advertising

Who we share your information with

  • Insurance carriers, wholesalers, aggregators, and managing general agents we submit your application or quote request to
  • Service providers that support our work: comparative rating platforms, agency management systems, e-signature and document platforms, and consumer reporting agencies used for underwriting
  • Claims adjusters and third-party administrators when a claim is open
  • Regulators and law enforcement, when required by law, subpoena, audit, or examination

We do not sell your personal information. We do not sell insurance leads.

Health information

Health information collected for life, disability, or health insurance is used only to obtain and service that coverage. We disclose it only to the carriers, reinsurers, and medical information exchanges named in the authorization you sign, and only for the purposes stated in that authorization. We do not use health information for marketing and we do not share it with our affiliates.

Sharing between Tetra companies

Tetra Insurance Agency LLC is affiliated with Tetra Wealth Management LLC, Tetra Home Loans LLC, and Tetra Tax LLC. All four are owned by David Rakecky.

We share your information with those affiliates only when you give us written authorization to do so. You are never required to use an affiliated company to buy insurance from us. Health information is never shared with affiliates.

Mobile information and text messaging

We will not share your mobile information with anyone. Mobile information and text messaging originator opt-in data and consent will not be sold, rented, or shared with third parties or affiliates for their marketing or promotional purposes. This exclusion applies to every category of sharing described anywhere else in this policy. Text message content and mobile opt-in data are disclosed only to the messaging platform vendors that deliver and archive our messages on our behalf, and to regulators when required by law.

How you opt in. You give consent by checking a box on a form, by signing a document that includes a consent line, by verbally agreeing on a recorded or documented call, or by texting us first. Consent to receive text messages is never a condition of getting a quote, buying a policy, or any other service from us.

What we send. Quote and application status, document requests, appointment and renewal reminders, claims updates, coverage and market information, promotional offers, and account or security notifications.

Message frequency varies based on your policy activity. Message and data rates may apply.

How to stop. Reply STOP to any message to opt out. Reply HELP for help. You can also email or call us using the contact information below.

We do not send health information by text message.

How we protect your information

We maintain a written information security program. Safeguards include encrypted storage and transmission, multi-factor authentication on business systems, a password manager, access controls limiting access to the owner only, locked physical storage for paper records, and secure destruction of records at the end of their retention period.

We ask that you never send a Social Security number, account number, or medical information to us by plain email or text. Use our secure document portal or our e-signature platform instead.

Data breach notification

If your unencrypted personal information is acquired without authorization, we will notify you without unreasonable delay in accordance with Michigan's Identity Theft Protection Act, MCL 445.72, and we will notify the Michigan Department of Insurance and Financial Services as required under Chapter 5A of the Insurance Code.

How long we keep your information

We keep insurance files and related records for the period required by Michigan producer recordkeeping rules and by the carriers we place business with, and longer where a specific rule requires it. Text message records are retained under our compliance archiving policy. We securely destroy records once the retention period ends.

Your choices

  • Opt out of text messages by replying STOP
  • Opt out of marketing email using the unsubscribe link in any email
  • Ask us not to call you and we will add you to our internal do-not-call list
  • Request a copy of your information, or ask us to correct something that is wrong, by contacting us. Under MCL 500.541 you have the right to access and request correction of personal information we hold about you, and to have a statement of disagreement filed if we decline
  • Turn off cookies in your browser settings

Some information we are required by law or by carrier contract to keep even after you ask us to delete it.

Children

Our services are for adults. We do not knowingly collect personal information from anyone under 18, except information about minor household members or beneficiaries that an adult applicant provides to us.

Changes to this policy

We may update this policy. The effective date at the top shows when it last changed. Material changes will be posted here before they take effect, and we will deliver an updated notice to active clients as required by MCL 500.525.
